Forms and Information

The 2025-2026 HomeCampus.com registration will OPEN on May 19, 2025.

Athletic clearance is exclusively online @ Homecampus.com

All athletes planning to participate in upcoming summer camps, conditioning, tryouts or on an athletic team during the 2025-2026 school year must be cleared through HomeCampus.com. A new physical exam and athletic clearance is required each year.

The first date of accepted physical exams for the new school year will be May 19, 2025. Physical exams dated prior to May 19, 2025 will not be accepted for the new school year.

Once you have completed the application, you will receive a notice notifying you that the application has been submitted for review. All correspondence will be via email regarding your status.
