Forms and Information

For the 2025-2026 school year, athletic clearance will once again be done exclusively online @ Homecampus.com

The OPEN Registration date for HomeCampus.com 2025-2026 is coming soon

The 2024-2025 HomeCampus.com registration will OPEN on May 18, 2024.

This will also be the first date of accepted physical exams for the new school year. Physical exams dated prior to May 18, 2024 will not be accepted for the new school year.

All athletes planning to participate in summer camps, conditioning, tryouts or on an athletic team during the 2024-2025 school year must be cleared through HomeCampus.com. A new physical exam and athletic clearance is required each year. The online application includes contact information, emergency contact information, medical history, various waivers, and a place to upload your physical form and the statement of consent and signature confirmation form.

Once you have completed the application, you will receive a notice notifying you that the application has been submitted and then within a few days you should get an email notifying you that the application has been cleared or denied. If it is denied, the note will include what is missing in order to get cleared.
